PLEASE READ: This registration form is designed to facilitate the scheduling of Flu and COVID Vaccination appointments for employees and students of the University of Maryland Baltimore Campus and University of Maryland Medical System. PLEASE NOTE: At this time COVID booster doses will be administered to CURRENTLY ENROLLED UMB STUDENTS ONLY based on CDC guidelines. (No Employees/ Faculty members at this time). When registering for your appointment please use the name listed on your insurance card as this will prevent accidental duplication of medical records. Please bring your insurance card and photo ID to your appointment.
