PLEASE READ: This registration form is designed to facilitate the scheduling of Asymptomatic COVID-19 Testing for employees and students of the University of Maryland Baltimore Campus and University of Maryland Medical System. When registering for your appointment please use the name listed on your insurance card as this will prevent accidental duplication of medical records. Please bring your insurance card and photo ID to your appointment.
